How do I pledge to donate to the tissue or Eye Bank?
First, talk with your family. Make sure they know of your wishes. This is the first and
most important step of all.
You can sign the back of your driver’s license to declare your wishes. Also, share your
wishes with your family. Keep a copy with you and make sure your family knows your
wishes. In Indiana, according to the Uniform Anatomical Gift Act, a priority list is used to
determine who will make the decision regarding your wishes. The priority list is:
1. Your spouse
2. Your children 18 years or older
3. Parents
4. Grandparents
5. Siblings 18 or older
6. Legal guardian
So be sure to talk with these people and let them know where to find a copy of your form.
Prepare a few more copies of the form and share them with your family, and keep one
with your will, insurance or other important papers.  Give one to your family physician for
your files. If you pre-plan your funeral, give a copy to your funeral director.
Communicating with your family is the crucial step. Use this form to share your decision to
donate life, because knowing your wishes can be a great comfort to your family.

Indiana’s Donor Choice Law
Indiana’s Donor Choice law allows you register in a database to declare your
wish to donate organs or tissue in the event of your death. You can register at
www.donatelifeindiana.org.
Again, remember that the most important thing you can do is talk with your family about
your wishes. Why is this so important? Because in the absence of clear documentation or
evidence of your own wishes, it will be left up to your family to decide in the event of your
death.
